It sounds like something is seriously wrong with your Photoshop... Try

They go through a long list of troubleshooting "artifacts" which is
what I think you may have. Hopefully something in there will solve
your problem.

Failing that I would either reinstall Photoshop, or ring/email Customer 
Services
